The Geriatric Evaluation and Management (GEM) at home program focusses on the health and care of people over the age of 65. A team of health and allied health professionals, including a Geriatrician, provide care and treatment to assist in managing conditions that occur during the aging process such as falls, incontinence, cognitive issues and problems with mobility and frailty.
Referrals
A specialist referral is required from your GP. The referral will be assessed by the geriatrician and staff will contact you to organise your home visit if you are assessed as being suitable for the program.
The Gem at Home program can service clients that reside within 30kms of Warrnambool. Contact the Discharge Support and Liasion team for any queries about the program.
