Suicide Prevention

To prevent suicides and reduce the devastation it causes, it is important that our communities get the support they need. Our services aim to reduce the risk of suicide, to support people at risk of taking their own lives and to provide support to families and loved ones who may be bereaved by suicide.

Hospital Outreach Post-suicidal Engagement (HOPE) Program:

The HOPE Program aims to provide assertive outreach support to individuals at the point of discharge from hospital, that are identified as at-risk of suicide.

The HOPE program provides intensive, person-centred support which is tailored to the unique needs and circumstances of the individual. Those eligible for HOPE will be
contacted within 24-hours of hospital discharge. HOPE engagement can occur via outreach and clinic based contacts, with the provision of brief therapeutic interventions and wellbeing support.

The HOPE program also supports the person’s carers and families.

The Mental Health Referral Services Team are the gateway to the HOPE program and accepts referrals from the regional Emergency Departments, a self-referral and referrals from GPs by calling the Mental Health 24/7 Centralised Triage phone advisory line on 1800 808 284.
