Specialist Physicians Clinic

Specialist clinics at South West Healthcare are an important link between care in the hospital and transition back into the community. They provide ongoing specialist management of chronic and complex conditions.

Specialist Physicians Clinics also called the Specialist Outpatient Clinics,  are public outpatient services. These consultations have no out of pocket cost for all Medicare card holders.

These appointments are to support your GP with providing medical specialist input to help with your medical condition.

The treating team at Specialist Physician Clinics consists of consultant specialist physicians, medical registrars (physicians in training), nurse practitioners, dietitians and other allied health professionals.

The clinics provide diagnostics, assessment, investigation, and management of complex medical conditions to help you and support your GP with your ongoing care.

You will be involved with all your treatment decisions. We will work with you to ensure you make choices regarding your care. We are committed to employing treatment plans best suited to individuals.

Discharge

The Specialist Outpatient Clinic will provide outpatient management and review until your health problem(s) are stabilised and a management plan has been created.

At discharge from The specialist clinic, a referrals to Community Health Services may be initiated to assist you and your primary healthcare provider (GP) with ongoing support for your medical condition.
