South West Healthcare Pharmacy provides comprehensive medication services to a range of patients and carers as needed for their best care.

How to access this service

Hospital pharmacy services are provided to patients during their admission and at discharge. However we can also visit patients as required upon referral by another member of the hospital team. Please ask if you would like this service.

Outpatient prescriptions may be presented to the Pharmacy during business hours. While we will process prescriptions as soon as possible, discharge prescriptions for inpatients are processed as a priority in the mornings. Please keep in mind that some items will need to be purchased or prepared especially for you, so where possible we ask for 3 days notice before collection.

Hospital pharmacists are part of the healthcare team, and ensure the safe and effective use of medications during your stay. A pharmacist may visit you as part of your inpatient stay to ask you about what medications you usually take when you are home. We may also ask to look at any medications you have with you as these are a great source of information.

Health Professionals

The services provided by the Pharmacy Department include:

  • Prescription review, dispensing and patient education for medications required upon discharge and for outpatients.
  • Clinical medication review (including documentation on Medication Management Plans) for inpatients
  • Medication advice for doctors, nurses and other healthcare professionals
  • Preparation of specialised medicines for infusion including chemotherapy and infusions for the Day Stay Unit.
  • Education of pharmacy students and interns

Patient information

  • What to bring

    When going into hospital, where possible, please bring the following with you:

    • Your current medicines
    • Your updated medicines list
    • Your allergy history
    • Your Medicare card, Concession card, Safety Net card (if applicable)
    • Details of your local doctor and local pharmacy

    When dispensing your prescription Pharmacy will calculate the cost of your prescription. It is appreciated if accounts can be settled when collecting your medications.

  • After your appointment

    There is no charge for medicines supplied to you during your stay in hospital. When you are discharged from the hospital the doctors write a prescription which is dispensed by the hospital Pharmacy.

    Prices are based on what PBS category you are usually in – General, Concession, Repatriation or Entitlement. The amount you pay, even for non-PBS medications, can contribute to your safety net threshold for the calendar year.

    If you have any of the following cards, please bring with these with you when presenting your prescription to the Pharmacy Department:

    • Medicare card/ Health Care card/ DVA card/ Safety Net card/ Pensioner concession card.
