Heart Failure Services

The goal of our Heart Failure Service is to improve Heart Failure management and the quality of life of people living with it. Working together with Specialists, General Practitioners and Medical Teams to ensure people with heart failure get prompt follow up, medication optimisation, education about self- management and a referral to community nursing and allied health programs to maintain support.

Specialist Physicians Clinics are public outpatient services with no out of pocket cost for consultations.

Within our clinic, the Heart Failure Nurse Practitioner is in charge of nursing clinical leadership. Our Heart Failure Nurse Practitioner provides direct clinical treatment for clients with heart failure, including assessment, ordering diagnostic investigations, medication prescriptions and administration, and referral to other health experts, in collaboration with our Specialist Physicians.

Patient Information

  • Who do these clinics care for?

    Individuals with a diagnosis of heart failure.

  • How do I access this service?

    Urgent care is available after an evaluation in the Emergency Department.

    For all other care must be arranged by your regular GP, who will provide referral to Specialist Physicians Clinic.

  • What do I bring to an appointment?

    You should bring a signed Consent and Registration Form. This will be sent to you by SWH after your appointment has been booked.

    You should bring an up-to-date list of all your medications.

    You should bring a list of questions you may have regarding your condition.

    You may bring a family member/support person, if this is helpful.

  • Who do these clinics care for?

    Individuals with a new or existing diagnosis of Heart Failure requiring assistance with heart failure management inclusive of medication titration and/or improve self-management via specialist education.

  • How do I prepare my patient for an appointment?

    Please let your patients know that, once received, their referral will be triaged, and they will be contacted by Southwest Healthcare to book an appointment.

    The physician will write a letter to the referring doctor after the appointment to update them on what occurred at the appointment.

    Please give your patient an up-to-date list of their medications, relevant recent pathology results, imaging reports and most recent trans thoracic echocardiogram.
