COVID Remote Patient Monitoring information for GP's

South West Health Care provides a Remote Patient Monitoring (RPM) Service for people living in the Warrnambool City, Corangamite, Moyne and Glenelg Shires who are COVID positive (through either PCR or RAT) and who require medical monitoring while isolating at home.

The service provides phone based monitoring based on the following criteria:

  • Individuals who have tested positive within 7 days of a positive test (in acute illness phase)
  • Moderate symptoms with the potential to deteriorate, or with additional health concerns including the following;
    • Obesity, cardiac disease, diabetes, renal failure, liver failure, palliative stage cancer, asthma, chronic respiratory conditions, stroke, dementia and other neurological conditions.

Individuals eligible for the program must be symptomatic for COVID-19 and be of medium or high risk of deterioration. See attached Department Stratification Criteria.


The RPM Service

Patients are provided with a pulse oximeter and are registered on a web based platform so they are able to self-report their vital signs (pulse, oxygen saturation). Nursing staff will have daily or twice daily phone contact to monitor their condition and provide support and advice until the symptoms resolve or will escalate to a medical review/intervention if the patient’s condition deteriorates.


Patients are referred to the service through the Department of Health Vic. via one of two COVID positive pathways:

  • A positive PCR test will automatically put into the system and patient will be contacted by the Department.
  • A positive  on a RAT,  patients are required to self-report, the link on the page takes you to a reporting form, the form can also be completed by others e.g. parent, spouse. GP, alternatively patient can call the 1800 675 398.
