On top of our Camperdown Hospital increasing its range of surgical services this year, Manifold Place Community Health has expanded its nursing and allied health services.
Locals are already reaping the benefits of Manifold Place having been awarded a Western Victoria Primary Health Network (PHN) contract to improve access to primary health services for people living in and around the area, from Mortlake to Cobden to Peterborough to Timboon.
These services include diabetes education, dietetics, exercise physiology, occupational therapy, physiotherapy, podiatry and social work and counselling. And the spotlight’s firmly focused on slashing the region’s alarming diabetes and heart disease rates.
‘Our multi-disciplinary team of clinicians is growing,’ says Manifold Place manager Rebecca Van Wollingen. ‘The PHN funding has allowed for the recruitment of new staff and increased the hours of some of our existing staff.
‘The funding allows us to provide services for anyone who has a diagnosis of diabetes or cardiovascular disease, including hypotension, congestive cardiac failure and angina. There is no age limit and the services on offer go way beyond dealing merely with the actual diagnosis. For example, a client who has a diagnosis of diabetes or cardiovascular disease is eligible to meet with an occupational therapist to consider the need for a handrail in their shower at home due to poor mobility.
During clients’ episodes of care they’ll also be invited to participate in surveys about the care they’ve received for their chronic condition and, with their consent, will have metabolic parameters measured. This data will contribute to a wider health survey being completed by the Western Victoria Primary Health Network to determine successful interventions for chronic disease management. It will also map clients’ experiences of their chronic disease management.’
